To access our Individualized Passport Supports, you must have received Passport funding through the Developmental Services Ontario (DSO).

Please note:

All applications for Ministry of Children, Community and Social Services (MCCSS) funded adult (18+) programs must go through Developmental Services Ontario.

Individualized Passport Supports

Passport is a Ministry of Children, Community and Social Services (MCCSS) program that provides funding to adults (18+) with a developmental disability to participate in their communities and to provide caregiver respite services.

We support hundreds of people each year to use their Passport funding to connect with their communities

We begin by working with you to create a personalized plan and budget based on how you’d like to use your Passport funding at Community Living Toronto. From there, supports are tailored to your goals and may include help participating in community activities, coordinating experiences, exploring social, cultural, recreational, or athletic opportunities, registering for programs, purchasing tickets or supplies, and building skills that support independence, employment, or volunteering, plus more, depending on your needs.

Individual support is available Monday–Friday, 9:00 AM–4:30 PM EST to help you access the services and supports you need.
